To begin your child’s enrolment, simply complete the form below. This will direct you to a link where you can download the Application Form. Once completed, please email the form, along with your child’s birth certificate and any relevant documents, to enrolments@cms.catholic.edu.au. There is a $25 application fee, which can be paid over the phone.
Alternatively, you can visit the Front Office to collect a physical copy of the Application Form. Once completed, return the form with the necessary documents and pay the $25 application fee at the office.
As part of the enrolment process, we require several identification and official documents. Please bring the following (as applicable):
